The Permanente Federation 2025 Annual Report
A year of convening and collaborating to accelerate change
In a year that saw shifts in federal health care policies, The Permanente Federation brought together physician leaders and experts to work collaboratively with Kaiser Foundation Health Plan and Hospitals to strengthen the core mission of Kaiser Permanente in providing safe, affordable, high-quality care to more than 12.5 million members.
The Federation’s work advanced the Permanente Medical Groups’ ability to implement new technologies in patient care, improve care delivery for conditions such as cancer and maternal health, and accelerate change in key priorities such as patient access to care and overall experience, and quality measures for patients, including those covered by Medicare. The Federation also supported and coordinated the organization’s response to changes in public health policy, advocating for patients and demonstrating Kaiser Permanente’s leadership in health care.
Quality remains the north star for Permanente Medicine
The Federation’s quality and safety work fosters collaboration across Kaiser Permanente, shapes care delivery based on clinical expertise and evidence, spreads innovations and best practices, and provides medical education to support up-to-date clinical knowledge. In 2025, the Quality team saw the transition to a new chief quality officer at the Federation, and a refresh of the care philosophy that guides the delivery of Permanente Medicine.


Hawaii Permanente Medical Group
Hawaii Permanente Medical Group (HPMG) celebrated 65 years of caring for Hawaii in 2025, building on a history of clinical excellence in the state. HPMG advanced surgical and specialty care through expanded robotic surgery programs, new bronchoscopy innovations, and enhanced fluoroscopy technology. These advancements supported multiple medical and surgical specialties, including vascular surgery, head and neck surgery, ob-gyn, pulmonology, and general surgery, expanding access to advanced procedures across Hawaii.
To address Hawaii’s growing physician shortage, more than 480 medical students, residents, and fellows completed over 1,000 clinical rotations and shadowing experiences under the mentorship of 300 HPMG physicians.
Additionally, the medical group continued to grow the next generation of locally rooted clinicians through mentoring programs like “Dr. Me” for elementary students, Maui Career Exploration Days for high school students, and Native Hawaiian health electives designed for medical residents seeking deeper understanding of indigenous health practices and principles.


Mid-Atlantic Permanente Medical Group
Mid-Atlantic Permanente Medical Group (MAPMG) physicians from across the region received more than 700 “Top Doctor” recognitions in 2025 across a number of publications, reflecting the trust they’ve earned in the communities they serve. Kaiser Permanente of the Mid-Atlantic States also continued to set the national standard for quality care, earning 5 out of 5 stars from the NCQA (National Committee for Quality Assurance) for its commercial health plan. This marks the seventh consecutive year the Mid-Atlantic’s health plan has achieved the highest possible rating. Physicians were also continuously recognized by patients for providing exceptional high-quality care. With an average Google rating of 4.9 stars and more than 18,000 5-star Google reviews, MAPMG physicians continued to stand among the most highly regarded in the region throughout 2025.
An eye on innovation to enhance care delivery
As Kaiser Permanente continues to explore advanced technologies in health care, the Federation partnered with the Permanente Medical Groups and Health Plan to ensure technologies met clinical needs and helped markets and medical groups prepare for changes.



Northwest Permanente
Northwest Permanente (NWP) delivered strong results in 2025 across access, quality, and operational excellence.
Through Get Care Now — Kaiser Permanente’s digital same-day care platform — NWP reduced the number of patients unable to quickly find an available appointment by 51% year over year.
Based on national benchmarks, including HEDIS, NWP continued to excel in several key quality measures such as blood pressure and diabetes control, while adolescent immunization rates exceeded the 95th percentile nationally.
For the 10th consecutive year, NWP physicians received the most “Top Docs” rankings of all health systems in the Portland metro area. Physician engagement scores made significant improvements year over year, supported by ongoing efforts to reduce documentation burden and improve practice efficiency. These efforts also contributed to the medical group earning Silver-level distinction from the American Medical Association through its Joy in Medicine Health System Recognition Program, honoring health systems that demonstrate a commitment to advancing physician well-being and reducing burnout.


Southern California Permanente Medical Group
Southern California Permanente Medical Group (SCPMG) demonstrated its strength, resilience, and commitment to community in response to the Eaton and Palisades wildfires in January 2025. Physicians and employees provided over 6,000 volunteer hours to support impacted communities and first responders.
A new Sports Medicine Center was also opened at Intuit Dome, home to the LA Clippers, where SCPMG physicians provide high-quality care to professional athletes and local residents.
The medical group received global recognition for its Kaiser Permanente Intelligent Navigator tool, which was named by European Medical Journal as one of the top 10 global scientific innovations of 2025.
For the 18th consecutive year, Kaiser Permanente Southern California earned the highest clinical quality scores from California’s Center for Data Insights and Innovation. The region also achieved top NCQA ratings across all lines of business, including 5-star ratings for its commercial and Medicare plans — and the Centers for Medicare & Medicaid Services awarded 5 out of 5 stars to KP’s Covered California health plan.
Working toward a sustainable future
Various parts of the Federation advanced work that supports Kaiser Permanente’s mission of providing affordable, high-quality care to members. During challenging economic times for health care, efforts to reduce costs, streamline efficiencies, and maintain affordability help to ensure members’ ability to receive care and to sustain the organization’s value-based care model into the future.


Lifting the physician voice to improve Kaiser Permanente and health care
The Federation supported physician perspectives in influencing various focus areas inside and outside of Kaiser Permanente. Within the organization, the clinical point of view allows Kaiser Permanente to achieve its integrated approach to care, and externally, our physician leaders provide a unique perspective to physician-led medicine that emphasizes value-based care.


The Permanente Medical Group
The Permanente Medical Group (TPMG) opened 13 new urgent care centers in 2025 to expand access to care. Virtual care services through the Virtual Medical Center — including e-visits and secure messaging — helped address 5.7 million patient concerns in 2025, resolving many routine and urgent needs without requiring an in-person visit.
TPMG also advanced care for patients with complex needs by expanding Care Plus, a model built around multidisciplinary teams that provide coordinated, proactive care to help patients improve their health and quality of life while reducing emergency visits and hospitalizations. Growth in ambulatory treatment centers and home-based care programs further supported patient-centered care beyond the hospital setting.
Kaiser Permanente’s Medicare and commercial health plans in Northern California each received 5 out of 5 stars in NCQA’s (National Committee for Quality Assurance) Health Plan Ratings, and TPMG celebrated Gold-level recognition from the American Medical Association’s Joy in Medicine program, recognizing its commitment to physician well-being.


The Southeast Permanente Medical Group
The Southeast Permanente Medical Group (TSPMG) celebrated 40 years of care in Georgia. The anniversary year was capped off with a special town hall, welcoming past TSPMG executive medical directors to reflect on their tenure of leadership and the vision that continues to shape the organization’s future.
The medical group delivered strong performance in quality, access, and care experience. Permanente physicians helped Kaiser Permanente of Georgia to lead Kaiser Permanente in CAHPS national patient experience scores and ranked in the top half of health plans nationally for “Getting Care Quickly.” Kaiser Permanente of Georgia’s commercial plan received a 4.5-star rating — the highest for health plans in Georgia — while its Medicare plan maintained a 4.5 star rating. Care gap closures improved by 20%, and 18 specialties achieved at least a 10% improvement over 2024.
TSPMG also supported the launch of Permanente Edge, which is a framework of guiding care principles that help define Permanente Medicine and the practical application of Kaiser Permanente’s value-based care model.
