With challenges such as hospital capacity and an aging population\, the health care industry must look for innovative\, sustainable ways to deliver safe\, high-quality\, equitable care today and in the future. \nThe growth of hospital-at-home programs offers opportunities for higher patient satisfaction\, reductions in readmissions and hospital-acquired infections\, and the provision of wraparound services\, but questions remain about the role of care teams\, equity and accessibility\, and taking such programs to scale. She leads operations for this physician-led\, self-governed\, multispecialty group practice\, which provides care to more than 620\,000 members of Kaiser Permanente in Oregon and southwestern Washington. A general surgeon who’s been with Northwest Permanente since 2006\, Dr. Giswold has served in many leadership roles\, including associate medical director for hospitals and post-acute care\, director of operations\, and Northwest Permanente board director. Throughout the COVID-19 pandemic\, she co-led the regional response leadership team for Kaiser Permanente of the Northwest and represented the Kaiser Permanente Advanced Care at Home program for a group of regional hospital chief medical officers. \n  \n  \n \nDavid Levine\, MD\, is a practicing general internist and clinician-investigator at Brigham Health and Harvard Medical School. He is active in clinical and translational research with a focus on optimizing quality time at home by shifting care to home\, decentralizing care\, expanding digital health technology\, and measuring the quality and experience of health care. He is medical director for strategy and innovation of the Brigham Health Home Hospital and co-chair of the Hospital at Home Users Group. Dr. Levine sees his role as helping patients achieve the right care at the right time in the right place by designing\, implementing\, and evaluating innovative interaction spaces among the care team\, technology\, caregiver\, and patient. \n  \n  \n \nMichael Maniaci\, MD\, is enterprise physician lead for the Advanced Care at Home program as well as current medical director of Mayo Clinic Hospital in Florida. His previous leadership positions with Mayo Clinic in Florida include chair of the Division of Hospital Internal Medicine\, associate chair of inpatient practice for the Department of Medicine\, associate program director for the Internal Medicine Residency Program\, associate medical director of the Mayo Clinic Multidisciplinary Simulation Center\, and medical director of patient experience. He is also associate professor of medicine in the Mayo Clinic College of Medicine and Science. \n  \n  \n  \n \nSarita Mohanty\, MD\, is president and chief executive officer of The SCAN Foundation\, one of the largest foundations in the United States focused on improving the quality of health and life for older adults. Its mission is to advance a coordinated and easily navigated system of high-quality services for older adults that preserves dignity and independence. Previously Dr. Mohanty served as vice president of care coordination for Medicaid and Vulnerable Populations at Kaiser Permanente. She also had past roles as assistant professor of medicine at University of Southern California; chief medical officer of COPE Health Solutions\, a health care management consulting company; and senior medical director at L.A. Care\, the largest U.S. public health plan. \n  \n  \n \nStephen Parodi\, MD\, (host/moderator) is executive vice president of External Affairs\, Communications\, and Brand at The Permanente Federation. He is also as an associate executive director for The Permanente Medical Group with responsibility for hospital operations\, inpatient quality\, patient safety\, care of complex and special needs populations\, skilled nursing facilities\, home health\, hospice\, and advanced analytics. Additionally\, he is helping to spearhead the new Advanced Care at Home Coalition.
