SUMMARY:Hospital @ Home Leadership Summit
DESCRIPTION:While health care challenges in the first wave of COVID-19 sparked interest\, the value proposition of in-home hospitalization alternatives is fueling the model’s momentum as we move out of the public health emergency. Although\, early in the pandemic\, large academic medical centers and health systems adopted the shift through the government’s Acute Hospital Care at Home waiver\, payers are increasingly leaning into strategies that include high-acuity health care in the home. However\, to create sustainable reimbursement mechanisms\, we first need a better definition of the value provided by this model of care. Permanente physician leader Vivian Reyes\, MD\, join the panel\, “The Payer’s Perspective: A Broader Definition of Value\,” which will explore the considerations from the perspective of a payer including how to clinical comfortability (quality and safety)\, member experience of care\, and figuring out mundane logistics like billing procedures. Join this panel on June 6\, at 2:30 p.m. ET. \nLearning objectives\n\nDiscussing the key metrics to better understand bottom line value \nExploring the payer’s perspective on the value of high-acuity healthcare in the home\, including quality and safety; measuring medical cost savings; and the value beyond direct medical cost savings \nUnderstanding the obstacles to ongoing scaling of these programs  \nLearning more about the monthly reporting requirements including unanticipated mortality\, transfer to inpatient care at brick-and-mortar hospitals\, and volume of patients treated \n\nSpeakers\n\nVivian Reyes\, MD\, national physician lead\, Kaiser Permanente Care at Home\, The Permanente Federation; regional medical director\, The Permanente Medical Group\nPatrick Kneeland\, MD\, vice president of Medical Affairs\, DispatchHealth \nDavid Brumley\, MD\, MBA\, senior vice president and chief medical officer\, Fallon Health \nJatin Dave\, MD\, MPH\, chief medical officer\, MassHealth and Associate Professor\, Population and Quantitative Health Science\, UMass Chan Medical School

SUMMARY:Reuters Events™: Increase Women’s Access to Gender Specific Care and Preventative Care
DESCRIPTION:Poor access\, increasing levels of maternal morbidity\, and mortality as well as limited ob-gyn care team numbers. Is this the reality for American women?  \nWith resources having been directed away from women’s health over the past 3 years due to the global health emergency\, gender–specific care access among the female population in North America has drastically worsened every year\, causing a concerning spike in negative patient outcomes.    \nThis Reuters Events webinar looks at how health organizations can leverage digital tools to increase interaction between women and their care teams\, as well as maximize the potential remote monitoring initiatives have\, in battling the “OB deserts” crisis.  \nIn this webinar\, a group of experts in the women’s health space will share the blueprint on how to:  \n\n Increase women’s engagement with their own health by providing higher levels of interaction with their care team via a central platform.\n Reduce negative patient outcomes as a result of “OB deserts” and build in-house remote care programs of which cater to women in high-risk situations\, particularly pregnancies\, as well as low risk ones.\nSuccessfully scale out and operate women’s remote care programs across large catchment areas in such a way\, that maternal morbidity and mortality decreases regardless of the lack of OB providers.\n\nSpeakers\n\nKari L. SUMMARY:Modern Healthcare: 2023 Leadership Symposium
DESCRIPTION:Over the next decade\, nontraditional primary care physicians could capture around 30% of the U.S. market. For a decade now\, primary care has seen a steady stream of new business and clinical models\, including those from advanced primary care doctors\, retailers\, and payers. By 2030\, the space won’t look the same at all\, with expectations that nontraditional primary care physicians will make up as much as a third of the market. Payers\, health systems\, and investors will need to make strategic choices to determine where to play and how to win over the next decade. \nThe first discussion on September 15\, from 8 to 9 a.m. ET is entitled: “Primary Care 2030: Innovative Models That Will Transform the Healthcare Landscape.” \nSpeakers\n\nLeong Koh\, MD\, president and CEO\, Northwest Permanente \nMarc Watkins\, MD\, chief medical officer\, Kroger Health \n\nEvent website

SUMMARY:2024 Hospital at Home Healthcare Leadership Summit
DESCRIPTION:Healthcare leaders are turning to the hospital-at-home model to improve patient care. This approach allows acute-level care in patients’ homes\, reducing costs and enhancing outcomes. This conference explores successful hospital-at-home programs\, covering staffing\, the model’s current state\, and opportunities in home healthcare delivery. Vivian Reyes\, MD\, will be part of a solo presentation and a panel. Please see details below. \n  \nEvent website \n\nHospitals without Walls: The Future of Healthcare\nJanuary 22\, 2024\, 9:00 a.m. – 9:45 a.m. \nIn the wake of the pandemic\, doctors and designers are radically adapting their thinking about what a hospital can be and what it should deliver. The healthcare industry as we know it is undergoing a monumental transformation as providers increasingly connect with patients outside of a hospital’s four walls to enable more care and healing at home. Telehealth is quickly becoming a new best practice in the industry and accepted way to enhance the patient experience and improve patient outcomes\, all while potentially lowering the cost of care. Rather than serving as hubs of all care\, hospitals are fast becoming centers of excellence for specialized care or highly acute conditions\, and health care systems are turning to hospital-at-home models of care for other services traditionally delivered in person. With these shifts\, telehealth becomes the patient’s first point of access for urgent care\, consultations with a specialist\, self-care education\, chronic care management\, and more. While COVID-19 was certainly a catalyst for this transition\, telehealth utilization has stabilized at levels higher than before the pandemic – a clear sign that it is here to stay. This session will explore hospital-at-home models. \n  \nSpeaker\n\nVivian Reyes\, MD\, national physician lead\, Kaiser Permanente Care at Home\, The Permanente Federation\, and regional medical director\, The Permanente Medical Group\n\n\nPanel: Hospital at Home Success Stories\nJanuary 22\, 2024\, 2:15 p.m. – 3:15 p.m. \nHow can a healthcare organization provide high-quality hospital-level care to acutely ill older adults in their home that improves outcomes\, reduces health care costs\, and enhances the patient experience? Hospital at Home is an innovative care model being adopted by health care organizations that provides hospital-level care in a patient’s home as a full substitute for acute hospital care. The program is being implemented at numerous sites around the country by hospitals\, health systems\, home care providers\, and managed care programs as a tool to cost-effectively treat acutely ill older adults\, while improving patient safety\, quality\, and satisfaction. In this session you will hear success stories from leaders in the hospital at home field.  \n\nSpeakers\n\nVivian Reyes\, MD\, national physician lead\, Kaiser Permanente Care at Home\, The Permanente Federation\, and regional medical director\, The Permanente Medical Group\nVipul Bhatia\, associate chief medical officer\, WellSpan Health\nMichael J.
